Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to complete one of our Travel Health Questionnaires, you can either follow the link below to complete and send electronically or you can collect a copy from the surgery reception.  Once we have received this our nursing team will assess what is required for your planned travel.  Please then call the surgery a week or so later where the reception staff will be able to make an appropriate appointment for you.

Please do not leave this until the last minute, all vaccinations should be given at least 2 weeks before travel to allow them to work.
